Below you’ll find every significant accident investigation report of interest to the towing sector that is available online, updated as new reports become available. These should be used as a valuable lessons-learned resource to avoid repeating the same old mistakes over and again. If you’re absolutely determined to become infamous, at least show some originality and make a new mistake that we all haven’t seen many times before!

- The Barge Maria T dragging anchor while under tow by the T/V Scott Turecamo resulting in the severing of the 8-inch diameter Central Gas & Electric Natural Gas Pipeline in the Hudson River near Poughkeepsie, N.Y. on August 8, 1999.
- 5/7/02: Allision of the Tanker Hyde Park with the T/V Herman Pott and tow , and the subsequent allision with the Domino Sugar facility and the M/V Miss Leslie, at mile marker 90 on the Mississippi River on February 26, 1999.
